*Additional $30 -$60 surcharge for request for Senior / Principal therapists.
*Re-Assessment may be required for clients who have not attended for > 3months as condition may have changed.
Insurance Procedures:
Insurance claim procedures (for hospitalization, accident or travel insurance) vary across different providers. It would be advisable for you to check with your agent on the claim and procedures.
You may need a referral letter from your doctor/GP or specialist. You will need to pay for all your treatment sessions first, and subsequently claim from your insurer with our receipt.
Appointment Responsibility:
- Punctuality: Please be 10 minutes early before your scheduled appointment time
- Cancellation/ Postponement: Please provide us a courtesy call/SMS 24 hours before scheduled time. Repeated cancellation or no show, a full cost of your treatment / session will be charged.
- Late Arrival: Subjected to full fee of session and session will not extend beyond the schedule time.
